目的应用免疫组织化学染色的方法检测硒蛋白P在直肠癌组织中的表达,以探讨其与直肠癌发生的关系及临床意义。方法收集山西大医院2013年6月至2014年5月间行手术治疗并经病理证实的60例直肠癌组织、40例直肠腺瘤组织、40例正常直肠组织,应用SABC法检测上述组织中硒蛋白P的表达情况,结果依据阳性细胞百分率和染色强度进行评价,三组间样本率的比较采用无序行×列表?2检验(α=0.05),两组间样本率的比较采用独立样本?2检验(α=0.016 7),硒蛋白的表达与直肠癌临床病理参数的关系采用四格表?2检验(α=0.05)进行分析。结果硒蛋白P在正常直肠组织、直肠腺瘤组织、直肠癌组织中的表达阳性率分别为82.5%(33/40)、70.0%(28/40)、45.0%(27/60),组间差异有统计学意义(?2=15.680,P<0.001),癌组织与正常组织、腺瘤组织间差异显著(?2=14.063,P<0.001;?2=6.061,P=0.015);硒蛋白的表达与肿瘤大小、是否浸润浆膜有关(P<0.05),与性别、年龄、淋巴结有无转移、肿瘤细胞分化程度、TNM分期无关(P>0.05)。结论硒蛋白在直肠癌中低表达,对直肠癌的发生发展具有重要作用,有望为直肠癌的治疗提供新思路。 相似文献

This meta-analysis examined whether early decompressive craniectomy (DC) can improve control of intracranial pressure (ICP) and mortality in patients with traumatic brain injury (TBI).Medline, Cochrane, EMBASE, and Google Scholar databases were searched until May 14, 2015, using the following terms: traumatic brain injury, refractory intracranial hypertension, high intracranial pressure, craniectomy, standard care, and medical management. Randomized controlled trials in which patients with TBI received DC and non-DC medical treatments were included.Of the 84 articles identified, 8 studies were selected for review, with 3 randomized controlled trials s having a total of 256 patients (123 DCs, 133 non-DCs) included in the meta-analysis. Patients receiving DC had a significantly greater reduction of ICP and shorter hospital stay. They also seemed to have lower odds of death than patients receiving only medical management, but the P value did not reach significance (pooled odds ratio 0.531, 95% confidence interval 0.209–1.350, Z = 1.95, P = 0.183) with respect to the effect on overall mortality; a separate analysis of 3 retrospective studies yielded a similar result.Whereas DC might effectively reduce ICP and shorten hospital stay in patients with TBI, its effect in decreasing mortality has not reached statistical significance. 相似文献

Sequential Therapy Based on Evolvement of Patterns: A New Model for Treatment of Alzheimer’s Disease
In order to solve the problem of long-term (>9 months) efficacy in the treatment of Alzheimer''s disease (AD) by conventional therapy (CT), a staged and multiply-targeted sequential therapy based on the evolvement of patterns (STEP) was developed. Its main innovations include: (1) the time order of evolution of patterns defined by Chinese medicine (CM) in AD was found, that is, "the orderly pattern evolution starting from Shen (Kidney) deficiency, progressing to phlegm, stasis and fire, and worsening to severe toxin as well as functional collapse"; (2) the cascade hypothesis of Shen deficiency in AD and its sequential therapy based on Shen-reinforcing was proposed, that is, "reinforcing Shen in the early stage and throughout the whole process, resolving phlegm, activating blood and purging fire in the middle stage, detoxifying and replenishing vitality to stop the collapse in the advanced stage", and through meta-analysis, clinical drug use was optimized, thus the leap from "inferential selection" to "evidence-based selection" was realized; (3) the STEP regimen combined with CT maintained cognitive and behavioral stability in AD patients for at least 12 months, with cognitive enhancement and behavioral synergy after 9 months, and cognitive benefit was superior to CT at 9, 12, 15, 18, 21, and 24 months, respectively. The 2-year cognitive improvement rate was increased by 25.64% (P=0.020) and the cognitive deterioration rate was decreased by 48.71% (P=0.000). Among them, the cognitive and functional benefits of Shen-reinforcing therapy for very early AD (350 cases) for 1 year were better than the placebo (P<0.001), and the dementia conversion rate was reduced by 8.85% (P=0.002). The behavioral symptomatic relief of patients with vascular dementia received fire-purging therapy (540 cases) was superior to those received CT (P=0.016). These data suggested that the STEP regimen has synergistic effects on CTs at least in terms of cognitive benefit, and the earlier the use, the greater the benefit will have. Therefore, the STEP regimen should be considered as one of the clinical options, particularly for the dearth of effective pharmaceutical or immunological interventions that are currently available for AD. 相似文献

Hepatic myelopathy is a complication seen in patients with chronic liver failure with physiologic or iatrogenic portosystemic shunting. The main symptom is progressive lower limb dyskinesia. The role of the brain motor control center in hepatic myelopathy is unknown. This study aimed to investigate the gray matter changes in patients with hepatic myelopathy secondary to transjugular intrahepatic portosystemic shunt and to examine their clinical relevance. This was a cross-sectional study. Twenty-three liver failure patients with hepatic myelopathy(hepatic myelopathy group), 23 liver failure patients without hepatic myelopathy(non-hepatic myelopathy group) after transjugular intrahepatic portosystemic shunt, and 23 demographically matched healthy volunteers were enrolled from March 2014 to November 2016 at Xijing Hospital, Air Force Military Medical University(Fourth Military Medical University), China. High-resolution magnetization-prepared rapid gradient-echo brain imaging was acquired. Group differences in regional gray matter were assessed using voxel-based morphometry analysis. The relationship between aberrant gray matter and motor characteristics was investigated. Results demonstrated that compared with the non-hepatic myelopathy group, gray matter volume abnormalities were asymmetric, with decreased volume in the left insula(P = 0.003), left thalamus(P = 0.029), left superior frontal gyrus(P = 0.006), and right middle cingulate cortex(P = 0.021), and increased volume in the right caudate nucleus(P = 0.017), corrected with open-source software. The volume of the right caudate nucleus in the hepatic myelopathy group negatively correlated with the lower limb clinical rating of the Fugl-Meyer Assessment(r = –0.53, P = 0.01). Compared with healthy controls, patients with and without hepatic myelopathy exhibited overall increased gray matter volume in both thalami, and decreased gray matter volume in both putamen, as well as in the globus pallidus, cerebellum, and vermis. The gray matter abnormalities we found predominantly involved motor-related regions, and may be associated with motor dysfunction. An enlarged right caudate nucleus might help to predict weak lower limb motor performance in patients with preclinical hepatic myelopathy after transjugular intrahepatic portosystemic shunt. 目的 探讨汉族、维吾尔族抗结核药物诱导的肝损伤(ATDLI)患者IL-10分泌量,比较两民族之间存在的差异。方法 选取新疆维吾尔自治区胸科医院、石河子大学医学院第一附属医院确诊的ATDLI患者。采用双抗体夹心ELISA测定患者IL-10的分泌量。比较两个民族以及两个民族不同性别、不同肝损伤程度的患者血清IL-10的表达量。结果 共收集ATDLI汉族组患者100例,维吾尔族组患者135例。维吾尔族组ATDLI患者IL-10分泌量[(56.30±17.24)pg/mL]高于汉族组[(45.81±11.04)pg/mL],差异有统计学意义(P<0.0001)。汉族轻度、中度及重度肝损伤组患者血清IL-10水平分别为(44.73±9.10)、(47.39±10.58)、(49.52±13.21)pg/mL;维吾尔族轻度、中度及重度肝损伤组患者血清IL-10水平分别为(52.30±15.24)、(56.84±13.71)、(58.62±14.38)pg/mL。组间比较显示,IL-10的分泌量汉族、维吾尔族中重度肝损伤组患者均高于轻度肝损伤组患者(均P<0.05)。结论 ATDLI患者IL-10分泌量随肝损伤程度的加重而升高。ATDLI患者IL-10分泌量维吾尔族高于汉族。 相似文献
